Depending on the context of the conversation. They can mean the same but there is a subtle difference.

"Does it make sense?" asking the listener if what the speaker saying is LOGICAL to the listener. "Do you understand what I am saying" can also mean that. However, it can also be used in a situation where the speaker is speaking in a language that might not be understood by the listener.

You can also use "Does it make sense?" in this situation, but "Do you understand what I am saying?" is more accurate.

Either way, both are fairly understood. It's just a matter of specificity.

For example,
when i talk to little kids, "Do you understand what I am saying" is better?
when i talk to business person at the meeting, "Does it make sense?" is better?
