the "Feeling lost, but now and then" line is more related with the next lyrics being
" Live life, breathe air" and continuing with
" I know somehow we're gonna get there and feel so wonderful"
where it means even tho I'm feeling lost, now and then so from to time to time or from instance to instance, "I live life and breath fresh air, so I lose this sense of being lost and enjoy this life" because I have faith that I'm going reach where I wanna reach and now I feel wonderful!

if not then maybe this means:
"here I am once again" so this happened before so he/she/they felt lost before and now as well, the only difference is this is them feeling lost now, compared to then
so the use of "but now and then" suggests that the feeling of being lost is different from the last time. so they used the now and then to showcase that it didn't feel the same.
