Question about English (US)
Does this sound natural in English?
Today while I was at the grocery store, there was a lady in front of me who had no more cash to pay for her groceries so I paid them for her. She seemed embarassed of the situation but I told her that it's not a big deal. I know that good deeds returns. My only wish is to have my grandma who is sick still with me for the holidays.

At the very least it is easily understood, but not really natural. A corrected version would be:
"Today at the grocery store a lady in front of me at checkout couldn't afford all her groceries so I paid for her. She seemed embarassed, but I told her it was not a problem. I know that my good deeds will be repaid. My only wish is for me to be able to spend time with my grandma for the holidays'

Sounds good, consider using commas (,).
Today, while I was at the grocery store, there was a lady in front of me who couldn't afford her groceries, so I paid them for her. She seemed embarassed of the situation, but I told her that it's not a big deal. I know that good deeds returns.
My only wish is to still have my grandma, who is sick, with me for the holidays.

Today, when I was at the grocery store, there was a lady in front of me who had did not have enough cash to pay for her groceries. So I paid for them for her. She seemed embarrassed about the situation but I told her that it's not a big deal. I know that good deeds will return to me. My only wish is to have my grandma with me for the holidays, but she is still sick.

Very good. Just a few small preposition and punctuation changes.
"Today, while I was at the grocery store, there was a lady in front of me who didn't have enough cash to pay for her groceries, so I paid them for her. @MetRon Much better, though "I know that good deeds are returned" still sounds strange since it seems to imply somewhat that the person you do a good deed for will be the one to "return" it. Admittedly my previous example wasn't much of an improvement in that regard but I found it difficult to address so just changed it up a bit.
