This is probably more natural in a casual conversation.

being a doctor must be difficult -- is not wrong.
but more conversational.... would be: It must be hard being a doctor.
re: the sugar question... it's funny, because "sugar bowls" are not something you see on tables anymore. but, i would say this, as "I'm OUT OF sugar, I'll put it on the list".
when you say you are OUT OF something, that means you don't have any and you're going to get more.

@CorneliaAdams as @emtrivoli indicated, the use of MUST in that sentence is fine, and the use of "hard" is more natural than "difficult."
