It kind of depends on the person and what they may be going through at the moment. Perhaps they had some recent difficulty, I may ask “how have you been”
Many people say “how are you” as if they were saying “hello” . They don’t expect an answer. It’s common in offices and among superficial groups of people.

I get it that "How are you? " doesn't necessarily require an actual answer.
..and I wonder what about--> How are you doing ? --> How have you been?
