Thank you so much! When I looked it up on the internet it said that the first pancake basically means 'failure' so I thought people mostly use it as a noun. I also read that there are some parents who even say it to their first born and I felt quite sad...

I think it's mostly used as a noun, but you can use the whole expression to express disappointment in a failure!
But that is quite sad, people shouldn't say that about their children...
