cependant la réponse naturelle aux questions utiliserait plutôt "jamais".
Tu as déjà pris l'avion ?
Non je n'ai jamais pris l'avion.
on utilisera "pas encore" pour un évènement qui va se produire, comme dans la question 4.

"Are these questions and answers okay? "
From a linguistic point of view, not really OK, I would say.
As a French learning material creator and long-term French teacher, I really think this type of exercise/drill is more of a "break (slower)" to anyone studying.
The reason is quite simple: although the drill wants you to practice grammar to understand the core construction of the French language, it doesn't say that NO ONE would talk like that in French, neither in formal or informal context, neither orally or written.
As @projetsabotage said above, only "jamais" is the real correct answer most of the time.
1. Tu as déjà pris l'avion ?
= Non, jamais.
= Jamais.
