"On the spot" Means to do something without preparation, impromptu.
This is often used when you're in a social situation and someone asks you a question or wants you to perform a skill. This is called "putting you on the spot" because you were not ready or prepared to do that. It's usually quite uncomfortable.
"At once" and "Immediately" mean the same thing, to do something right away. The very same instant. As soon as you can.
