She looked back at me would mean she tilted her head or turned around to look at u and She looked at me again would mean shes continuously looking at u

She looked at me again means she looked at me a second time (after she first looked at me)
She looked back at me can mean two things: 1) she turned around to look at me or 2) she returned my look

Thereās a subtle difference between the two.
I told her I didnāt love her. She looked back at me in disbelief.
She returned my look and didnāt believe me.
I told her I didnāt love her. She looked at me again as if she didnāt believe me.
She stopped looking at me, turned away briefly and moved her head to look at me again.
