The first is just a simple question. The second implies that the person asking the question thinks that there is a reason to be nervous.
Aren't you nervous? I am. (or I would be if I was you)
Aren't you nervous? You have to perform in front of thousands of people!

The second example implies that the person asking would be nervous.
"Aren't you nervous? I would be if I were you."
The first example is simply a question.
