I'd probably say: He demonstrated great courage by telling the truth.

Here, "his" is not necessary. It's already shown that it is "his" by saying "he had." If he has it, then it's his, so then "his" becomes unnecessary to say.
So: "He had /great/ courage to tell the truth." is correct, but it sounds sort of stiff. "He had /a lot of/ courage to tell the truth." sounds better, but "He had /a lot of courage/ to /be able to/ tell the truth." is probably closer to what you want to convey, and the most natural, because his courage helps him tell the truth.
② doesn't sound very natural. In this case, you're describing how he told the truth. So it would be: "He told the truth /very courageously./"
"Very courageously" = "with great courage."
