So apparently there are seventeen forms of 'two' in Polish. They are:
1. dwa
2. dwie
3. dwoje
4. dwóch (or dwu)
5. dwaj
6. dwiema
7. dwom (or dwóm)
8. dwoma
9. dwojga
10. dwojgu
11. dwojgiem
12. dwójka
13. dwójki
14. dwójkę

I don't know when you must use on instead of another. I do not think about it, I just say :D

Technically, you mentioned three different words:
1) dwa - number
2) dwoje - collective number
3) dwójka - noun created from number
Numbers are declined and depend on the gender. They are used always, unless we`re talking about:
- children
- group of people of both genders
- young animals
- nouns having only plural form
For those things you use collective numbers, which are declined, but gender independent.
The nouns created from the numbers you can use always instead of numbers or collective numbers, but they usually sound less natural (not always). They are also declined, but gender independent.

Words 2 and 3 are used according to the pattern:
collective number/number-noun (declined) + noun (genitive).
Using casual numbers is much more complicated.

Now I realized that you didn't list forms of collective number version of "two", but you should have :-). So there is more forms of English two in Polish than you listed.
