Dipende dal contesto, ma varie interpretazioni possono essere:
- un uomo che non aveva paura di dire quello che pensava o fare quello che voleva
- un uomo molto abile in qualcosa
- di grande valore
Ovviamente è un'espressione volgare, da usare solo con gli amici.
Un'alternativa è "un uomo con la u maiuscola", ma usando questa seconda espressione puoi solo intendere "un uomo di valore"
Altri esempi:
- un musicista con le palle (= "molto bravo")
- una donna con le palle (= una donna molto coraggiosa / forte. Un po' sessista come espressione, pero' si dice così)

Per rimanere sul volgare, aggiungiamo che si può dire anche "un uomo con due coglioni/palle così" (accompagnato spesso da un eloquente gesto) o "un uomo coi controcoglioni", dove vediamo un curioso uso rafforzativo del prefisso "contro-".
