"Contact the clinic" is grammatically correct. I have never heard or used "contact to the [anything]" ever before.

"Contact to the clinic" is one thing. For example: I have contact to the clinic (this type of sentence is only rarely used)
"Contact the clinic" is a verb phrase. For example: I contact the clinic to schedule an appointment.
