Are they the same? I also want to know why the first one says vous twice.

The second one is incorrect, there should be 2 "vous": "Comment vous appelez-vous ?".
The difference is that the first form is more casual, the second one is more formal and almost never used in speaking, it's mostly used when you write a formal text.
They mean the same.
In an informal situation, we don't use "vous" but "tu", so it becomes "Comment tu t'appelles ?" (you could also technically use "Comment t'appelles-tu", it's grammatically correct, but it sounds a bit odd since "tu" is informal but that grammatical construction is formal).

"comment vous vous appelez" is correct but it's more a spoken form and more informal. "comment vous appelez vous" is more formal, grammatically right and use for the written form.
"Comment appelez vous... " means "How do you call..." and you ad "this or this", Exemple : "Comment appelez vous ceci ou ceci ?
