You order like in any other restaurant/bar. They tend to be crowded though, so try to make yourself visible (raise your hand, for example/make eye contact)

Well, first of all, "tapa" is actually a size of dish. It is the small dish. The medium one is "media ración" and the biggest is "ración". So here you have an example of a conversation (after already having ordered the drinks):
Camarero: ¿Saben ya lo que van a tomar?
Ana: Sí, yo quiero una tapa de ensaladilla y media ración de croquetas.
Dani: Para mí una tapa de patatas bravas y media ración de solomillo al whisky.
María: Para nosotros una ración de adobo para compartir...
Javi: Una tapa de patatas ali oli
Maria: Y yo otra
Camarero: De acuerdo, ahora se lo traigo
Though there are some dishes that have a standard size. Either they just come in tapa/media ración/ración (usually the two last), either they "don't have size" and you have to use the word "plato" (in the menu you'll see that like "platos" as the title). But that last is more common in restaurants than in bars.

It depends on the region, in some cities in Spain you just order a drink and the waiter serves it along with the tapa; sometimes you are asked which tapa you want, sometimes not

Depends on where you are.
Where I live we have a blackboard with the variety of the day, and when you order the drink the waiter asks you ¿qué quieres de pincho? and you choose what you want.
But in other places you have to pay for it, so you have to ask for it yourself. I've been in Barcelona and there you order your drink and tapa and pay both.
