If the lungs are in a jar or on a table "That is a patient's lungs." A x-ray is a copy. That is why "of" is used for statement "The X-ray is of a patient's lungs."

Second one is a bit strange. It kinda means "the x ray is literally a patient's lungs". Like, someone is using someone 's lungs to x ray people
