Question about Spanish (Mexico)

In many cases the "subjunctivo presente" is the same as the "imperativo afirmativo". For example hable, hablemos, hablen. What makes the distinction? How does the listener know which is the case? Or are they really the same. Is "Hable con su médico acerca de los riesgos personales del cáncer." using the subjuctive presente or the imperativo afirmativo?
