" Él mismo" se refiere a algo en específico, por ejemplo una persona, un objeto, por ejemplo: ese es el mismo hombre que vi ayer, "ese es el mismo pantalón que compre", y cuando usas LO MISMO, es algo que igual, algo repetido, por ejemplo: Qué estás viendo? - lo mismo de ayer, o tome lo mismo que ayer, en sí es algo que ya has hecho, o que ya tienes, alguna acción repetida, que es IGUAL...
Espero que te sirva.

When you use "lo mismo" it means that you're talking in plural and you are not specifing genders.
Nos gusta lo mismo/We like the same things.
And when you say"el mismo" you're being specific and you must talk about something that is singular and masculine.
Tenemos el mismo suéter/We have the same sweater.
