"I totally can't understand" is strange. You should use "totally" with positive sentences, not negative ones.
These three sentences have the same meaning:

@wonyil They are both technically correct (you should say "words," not "worlds"), but both sound a little unnatural to me.
When you don't understand something, I would say, for example:
(1 = less confused, 4 = more confused)
1. I'm sorry, I didn't understand everything you said. Could you say it again slower or using different words?
2. I'm sorry, I didn't understand some of the words that you used.
3. I'm sorry, I don't understand what you're saying.
4. Please slow down, I'm really confused! I can't understand what you're saying at all. Could you say it again slower?
